Baldness is not caused by excess testosterone as is commonly thought; nor is it inherited from your maternal grandfather. It’s caused by sensitivity to testosterone: an enzyme converts testosterone into a substance called dihydrotestosterone – which then causes the follicle to shrink and fall out. Male pattern baldness is inherited, but from both or either side of your genetic line. Why men go bald, we’re not sure – though there is a hypothesis that baldness was once a genetic advantage. In our age of appearances not so much. Of 17 male members of Cabinet, two are bald (Chris Grayling and Sajid Javid). Studies correlate baldness with depression; the term “psycho-trichological” is used to describe the feelings of disfigurement, social avoidance and anxiety disorders that often arrive with premature baldness.
What’s equally frustrating for women is that their hair loss is not as clearly understood as hair loss in men and, according to an article in www.dermentz.org, “..presents itself quite differently from the more recognizable male pattern baldness which usually begins with a receding frontal hairline that progresses to a bald patch on top of the head.” The same article also says that it’s uncommon for women to follow a male pattern unless there is excessive production of androgens in her body.
When it comes to a hair growth shampoo, you have two options: prescriptions or over-the-counter treatments. Prescription shampoos like ketoconazole are anti-androgenetic, meaning they prevent testosterone and dihydrotestosterone (DHT), two hormones associated with hair loss by targeting hair growth at the site of the follicle and helping to interrupt the hormone cycle locally that causes thinning,” explains Dr. Schweiger. “Over-the-counter growth shampoos typically contain amino acids and antioxidants to provide the components to rebuild hair as well as reduce damage and inflammation.”
We've heard it all before: clients waking up one day in shock after discovering a coin-sized bald spot on their heads; women agonizing over the strands of hair they see on the shower floor; men looking for topical creams and shampoos to prevent the early onset of balding. Yet another company has made news this week for phase 2 trial progress. Concert Pharmaceuticals announced today that they have completed enrollment for their phase 2a trial using CTP-543 in alopecia areata. CTP-543 is an oral JAK inhibitor which acts on JAK 1 and 2, it’s also known as ruxolitinib. Concert’s version of ruxolitinib has been modified by the company’s proprietary deuterium chemistry technology which the company hopes will improve its effects on AA.
Things get interesting when we discover a patent which was filed by Sangamo in May 2017 titled “Targeted Treatment of Androgenic Alopecia.” As with virtually all patents, the lengthy text of the patent is difficult to read or to create a concise summary from. An intriguing aspect of this news is Sangamo works in several technology spaces, including previously mentioned genome editing and gene therapy, either would make an advanced type of hair growth therapy we have never seen before. One caveat to mention is the company’s pipeline does not currently display any indication for alopecia, meaning the therapy is not fully developed yet, so it will be some time before trials begin. We certainly hope to hear more from Sangamo Therapeutics as soon as possible about their interesting development for hair growth technology.
Niacin, also known as vitamin B3, is another vitamin that you can find in a lot of men’s hair loss shampoos. Like biotin, niacin and its derivatives are scientifically proven to increase hair growth when topically applied to hair growth areas over the course of several months. However, it's important to call out that this pilot study used female participants.
Two clinical trials have been ran as a proof of concept for Histogen. Terminal hair count and hair thickness noticeably increased after just 12 weeks. Due to this success, Histogen plans to conduct a Phase 1 Clinical Study in the United States. This will be an injectable which when injected into the scalp will stimulate dormant hair follicles and induce new hair follicle formation (think Botox but for hair follicles instead of skin cells). HSC660 is an ongoing female hair loss trial that will run for 22 weeks and a late stage (Phase 3 trial) for men has initiated in Mexico. It may not be a magic bullet, but it would sure be nice to have an alternative/supplement to Rogaine that actually stimulates growth.
