Contact immunotherapy. Another drug that can be administered for cases of alopecia areata is contact immunotherapy and is recommended for severe cases. Diphenylcyclopropenon (DPCP) is applied on the scalp every week, and the dosage of the drug is increased over time until a mild allergic reaction is observed, which signals that the drug is taking effect. Regrowth may be observed within three months from the beginning of treatment.

When you're investing considerable time and money on a mix of hair treatments and cures, the last things you should be doing are those that will only aggravate your condition. Likewise, when your hair has finally grown back, the last thing you would want is for you to go through another horrific episode of hair loss. Preventing further - or another case of hair loss, and stopping it before it actually happens should be your goal. 

Hair loss in alopecia areata occurs in three different patterns: patchy alopecia is circumscribed, oval-shaped, flesh-colored patches on any part of the body; alopecia totalis involves the entire scalp; and alopecia universalis involves the whole body. Evaluation of the scalp may reveal short vellus hairs, yellow or black dots, and broken hair shafts (which are not specific to alopecia areata). Microscopic examination of the hair follicles demonstrates exclamation mark hair (i.e., hairs that are narrower closer to the scalp and mimic an exclamation point; Figure 44). Nail pitting is also associated with alopecia areata.


2. Pyrithione zinc shampoo. Traditional volumizing shampoos will give the hair you have a lift so it looks fuller (we like the sulfate-free L’Oréal Paris EverPure Volume Shampoo, $8). But some research suggests shampoos with the antidandruff ingredient zinc pyrithione can mitigate hair loss that’s caused by conditions like dandruff, says Mirmirani. Try Head & Shoulders Deep Moisture Shampoo ($6), and use a conditioner without silicones — they can make hair appear limper, especially if it's applied near the roots (we like Love Beauty and Planet Coconut Water & Mimosa Flower Conditioner, $9).

While it may be easy to treat hair loss that's caused by stress or other physical or emotional shock, other types, especially those caused by medical conditions, require expert attention. Do not self-medicate. A medical test is necessary to determine the real cause of your hair loss. A medical diagnosis is important in order to rule out other factors that might be the cause of your hair loss. On the surface, it's easy to attribute hair loss to stress or the lack of sleep, but it is highly possible that the hair loss may be caused by an unknown or undetected condition related to hormones or genetics. Hair loss is often distressing and can have a significant effect on the patient's quality of life. Patients may present to their family physician first with diffuse or patchy hair loss. Scarring alopecia is best evaluated by a dermatologist. Nonscarring alopecias can be readily diagnosed and treated in the family physician's office. Androgenetic alopecia can be diagnosed clinically and treated with minoxidil. Alopecia areata is diagnosed by typical patches of hair loss and is self-limited. Tinea capitis causes patches of alopecia that may be erythematous and scaly and must be treated systemically. Telogen effluvium is a nonscarring, noninflammatory alopecia of relatively sudden onset caused by physiologic or emotional stress. Once the precipitating cause is removed, the hair typically will regrow. Trichotillomania is an impulse-control disorder; treatment is aimed at controlling the underlying psychiatric condition. Trichorrhexis nodosa occurs when hairs break secondary to trauma and is often a result of hair styling or overuse of hair products. Anagen effluvium is the abnormal diffuse loss of hair during the growth phase caused by an event that impairs the mitotic activity of the hair follicle, most commonly chemotherapy. Physician support is especially important for patients in this situation.
Minoxidil: This medicine is applied to the scalp. It can stop hairs from getting thinner and stimulate hair growth on the top of the scalp. The U.S. Food and Drug Administration (FDA) has approved minoxidil to treat hair loss. It is the only hair re-growth product approved for men and women. A dermatologist may combine minoxidil with another treatment.
Minoxidil is a similar treatment to finasteride and is also available in the UK as a generic or as the well known brand-name “Regaine”. Minoxidil can be bought over the counter, unlike Propecia or finasteride, and comes in the form of a lotion, foam or cream that is rubbed into the scalp every day. Minoxidil is less effective than finasteride (only 60% of men who used Regaine Extra Strength reported regrowth after 48 weeks) but is still a viable option for men seeking treatment. It is also suitable for use by women, and can be used to treat alopecia areata. However, as with finasteride, any beneficial effects will cease once treatment has stopped.
Hair practices. Our hair is one of the strongest and most elastic parts of our body. One strand of a healthy hair can be twice as strong as a copper wire of similar thickness. However, not all hair types are equal. Unfortunately, hair care practices and styling can lead to scalp damage and unnecessary pressure on the hair follicles, resulting in hair breakage and loss.
Two clinical trials have been ran as a proof of concept for Histogen. Terminal hair count and hair thickness noticeably increased after just 12 weeks. Due to this success, Histogen plans to conduct a Phase 1 Clinical Study in the United States. This will be an injectable which when injected into the scalp will stimulate dormant hair follicles and induce new hair follicle formation (think Botox but for hair follicles instead of skin cells). HSC660 is an ongoing female hair loss trial that will run for 22 weeks and a late stage (Phase 3 trial) for men has initiated in Mexico. Histogen founder Gail Naughton even went so far to reveal commercialization, “We’e in very late-stage negotiations with some huge retail partners,” she says. It may not be a magic bullet, but it would sure be nice to have an alternative/supplement to Rogaine that actually stimulates growth.

3. Nutriceuticals. Rogers recommends that her patients try Viviscal Dietary Supplements ($50 for a one-month supply) or Nutrafol ($88 for a one-month supply), both of which can often be picked up in a salon. The former contains marine-based proteins meant to beef up thinning hair. The latter is packed with antioxidants, like ashwagandha and curcumin, and boasts impressive clinical results.

Treatment for adults with less than 50% of scalp involvement is intralesional triamcinolone acetonide injected intradermally using a 0.5-inch, 30-gauge needle. Maximal volume is 3 mL per session.11 Treatment may be repeated every four to six weeks until resolution or for a maximum of six months. Local adverse effects include transient atrophy and telangiectasia.

However, ketoconazole is still not FDA approved for hair loss treatment, which means it cannot be endorsed or marketed as such. Put simply, ketoconazole likely curbs hair loss, but additional research is needed for the FDA to give it approval. While it is safe to use as a supplement to our top picks, we wanted to recommend products with as much scientific backing as possible. In fact, the Japanese government has recently committed to establish a new approval process for regenerative medicine products focused on accelerating approval timelines. As it turns out, hair loss is a big concern in Asian countries. (Also last year, Pantene introduced its Hair Regrowth Treatment for Women, which is 2 percent minoxidil.)

The most popular low-light laser therapy product is developed by Hairmax. They have a series of Hairmax Laser Combs. Treatment is applied weekly or more frequently. Notably, the Hairmax Laser Comb was the 3rd product to receive FDA approval for treatment of genetic balding. It followed Minoxidil and Finasteride. Though some scalps are very responsive to this laser treatment, others seem to only reap the benefit of slowing hair loss. Well there are two types of 5 alpha reductase. Type I and Type II. Whereas Propecia has only been shown to block type II, Avodart blocks both types. This effect overall lowers DHT levels even more, further reducing the risk of damaged hair follicles. However, it faces the same limitations as Propecia. It must be taken daily and may lose its effectiveness overtime.

Scientists from the Indiana University School of Medicine have for the first time created skin with hair follicles using mice stem cells. Research was led by Professor Karl Koehler. The team was able to grow both the epidermis and dermis layers of skin to create a realistic skin model. An interesting quote from Professor Koehler: “It looks like a little ball of pocket lint that floats around in the culture medium. The skin develops as a spherical cyst and then the hair follicles grow outward in all directions – like dandelion seeds.”
