The combination of silent suffering, public shame and poorly understood science makes hair loss sufferers easy prey – type “hair loss” into Google and you’ll see what I mean. Kobren tells me that he had to remove the personal messaging function on the Bald Truth message board, as users were being bombarded by scams. Meanwhile, the higher visibility of celebrity transplants means that baldness is at risk of being seen as a sign of poor self-care. Many treat surgery lightly – and enter into punitive financing deals. One of Kobren’s recent guests was The Only Way is Essex star Maria Fowler, who complained that surgery at the controversial KSL Hair in Glasgow left her with an unnatural hairline. “She ended up having a hair transplant because her fiancé was having one. She had always thought her hairline was too high. She went in like she was having her nails painted – and it destroyed her life.”

There’s a lot of misinformation, half-truths, and pseudoscience regarding hair loss, and there are also treatment programs that have been well-researched and tested in clinical settings. So, how do you find the difference? For starters, talk to the experts in the industry like dermatologists and general physicians about treatment programs. Avoid people advertising secret cures, all-natural remedies, and permanent fixes. If there was a way to stop baldness from happening, we’d all know about it already.
Other options include microneedling ($1,200 and up per treatment) and platelet-replacement therapy (also $1,200 and up per treatment), which are usually offered in conjunction. Your scalp will be numbed first so you don’t feel the pinpricks involved in microneedling. Some 30 million women in the United States have hereditary hair loss (compared with 50 million men), according to the American Academy of Dermatology, though that figure does not include the millions more who struggle with thinning hair because of pregnancy, menopause, stress and other health conditions. Researchers from UCLA in the lab of Jing Huang have recently shown that certain molecules which activate the cellular process known as autophagy also drive hair follicles into the anagen (growth) phase. The researchers studied different metabolite molecules and other molecules which are FDA approved and on the market as drugs. The most recognizable drugs from the study were metformin and rapamycin, one is a diabetes medicine and the other an immunosuppressant. Dr. Carlos Wesley, a hair restoration surgeon in Manhattan, said that women in his practice respond better to P.R.P. than men do, which may have something to do with the fact that women with genetic hair loss tend to have more inflammatory cells around the follicles. It’s also possible that some of Harklinikken’s users are women whose hair would have grown back even if they’d done nothing. Many women who arrive in a dermatologist’s office with prior diagnoses of female pattern hair loss actually have what’s called telogen effluvium. That’s a period of acute shedding of hair — meaning up to 60 percent of hair — three months after a triggering event like pregnancy, significant weight loss or starting or stopping hormone medications.

Anagen effluvium is usually reversible, with regrowth one to three months after cessation of the offending agent. Permanent alopecia is rare. In the nineteen-forties, a Brooklyn anatomist named James Hamilton studied prisoners in Oklahoma who, having been convicted of sexual assault, were castrated. Hamilton identified testosterone as the root of hair loss, and showed that men castrated before or during puberty did not go bald. He then injected groups of castrated adult men with testosterone and—duly, cruelly—watched their hair fall out.
In 1952, a New York dermatologist named Norman Orentreich invented hair plugs. He removed hair from the back of a patient’s head, where it still grew, and grafted it onto the front. In the decades since, the transplantation process has become more refined. Following the lead of the pioneering dermatologist Robert Bernstein, most doctors perform follicular-unit extraction; instead of crudely ripping up large parts of the scalp, they pluck and move individual follicular units.

Some treatments in development hold particular promise for women. Angela Christiano, a hair geneticist and Columbia University professor of dermatology, is hoping to begin clinical trials in a year or two on a procedure in which she dissects hair-follicle stem cells, grows them in the lab until she has several million, then injects them into the scalp, where, a very small study done with a human skin model has shown, they induce new hairs.

Hair transplantation involves harvesting follicles from the back of the head that are DHT resistant and transplanting them to bald areas. A surgeon will remove minuscule plugs of skin that contain a few hairs and implant the plugs where the follicles are inactive. Around 15 percent of hairs emerge from the follicle as a single hair, and 15 percent grow in groups of four or five hairs.
The general medical consensus around laser treatments — caps and combs alike — is that low-level laser light therapy stimulates the cells within the hair follicle. These devices may also increase cell metabolism to promote thicker and more durable hair shafts, something that neither minoxidil or finasteride can do.
