“Smelling” Receptor Keeps Hair Growing – Many of you may have noticed the headlines regarding sandalwood and hair growth over the past week. The research everyone is talking about comes from Ralf Paus and his team at the Monasterium Laboratory GmbH. For the record, Paus is also the main researcher behind the WAY-316606 hair growth discovery. This time Paus et al identified an olfactory receptor in hair follicles, OR2AT4,  which plays a role in regulating hair growth or inhibition. Olfacory receptors are responsible for detecting odors in cell membranes and provide the basis for our sense of smell, they do carry out additional functions though, as demonstrated by Paus. 
JW Pharma, a Korean biotech company that specializes in the Wnt pathway, has entered a cooperative agreement with the University of Pennsylvania to development a compound called CWL08006 for hair regeneration purposes. JW Pharma has a pool of Wnt targeting therapies, and apparently CWL08006 makes a great candidate for hair growth. Of course, Dr. George Cotsarelis of UPenn is involved, and the two teams plan to begin preclinical testing by 2019 (I’m a little surprised at that timeline for preclinical). One of the more interesting facets of the report, JW intends “to accelerate its commercialization by launching tests on the human body for cosmetics that utilizes the drug’s mechanism, next year.” If the candidate works well in the cosmetic tests, a version of it could be made available much sooner than the clinical drug pathway, but for now it’s not possible to speculate a time frame for that yet. Good news all in all. 

The Holy Grail remains a drug that will promote regrowth, but this might not be so far away. Earlier this year, Manchester University announced that an osteoporosis drug had been found to have “dramatic results” promoting hair growth when applied to tissue samples in pre-clinical trials. The resultant frenzy left the PhD student responsible, Dr Nathan Hawkshaw, a little dazed. “Every other week, something comes out about hair loss and it doesn’t generate as much media coverage as what I experienced,” he grumbles. He’s in this for the science – there aren’t many fields where you get to mess around with real human tissue – but such is the distress caused by hair loss and such is the potential customer base that interest is always high. 

Anagen effluvium is abnormal diffuse hair loss (usually abrupt) during the anagen phase due to an event that impairs the mitotic or metabolic activity of the hair follicle. The incidence of anagen effluvium after chemotherapy is approximately 65%24; it is most commonly associated with cyclophosphamide, nitrosoureas, and doxorubicin (Adriamycin). Other causative medications include tamoxifen, allopurinol, levodopa, bromocriptine (Parlodel), and toxins such as bismuth, arsenic, and gold. Other medical and inflammatory conditions, such as mycosis fungoides or pemphigus vulgaris, can lead to anagen effluvium.25

The TYK2/JAK1 inhibitor showed the greater efficacy, improving hair regrowth by 49.5 points on the Severity of Alopecia Tool scale, compared to an improvement of 33.6 points by the JAK3 inhibitor. However, Pfizer has apparently decided to move forward with its JAK3 inhibitor due to 2 adverse events in the TYK2/JAK1 inhibitor cohort during the trial. Pfizer’s JAK3 candidate, PF-06651600, was also recently granted Breakthrough Designation from the US FDA for treating alopecia areata. 

The only nonchemical option offered up by the dermatologists I spoke with — short of a surgical hair transplant or platelet-rich plasma therapy, which is like Kim Kardashian’s vampire facial but for your scalp — was the laser comb. First cleared by the FDA in 2009, the HairMax LaserComb is a handheld laser device that is designed to promote hair growth. As the manufacturer explains in a letter to the FDA, “The device provides distributed laser light to the scalp while the comb teeth simultaneously part the user’s hair to ensure the laser light reaches the user’s scalp,” which, in turn, stimulates the hair follicles.
Some immediately see a doctor for Rogaine or Propecia, seek out over-the-counter miracle baldness cures, or even look into hair transplants. The annual World Congress for Hair Research kicks off today in Kyoto, Japan. The Congress will go on until Friday November 3, 2017. The purpose of the event is to share presentations about different lines of research and product development regarding alopecia areata, stem cell biology, androgenic alopecia, cosmetic care, wigs, hair transplants, and more. Basically, all things hair industry. At these conferences a few companies typically will announce some news about their developments and progress. According to an article on Newsweek, the FDA-approved drug dupilumab was given to a 13-year-old alopecia sufferer to treat her eczema. The patient, who hadn’t grown hair on her scalp since she was two, suddenly grew a significant amount of hair on her head after continual use of the drug, a study in the journal JAMA Dermatology reports.
Another shampoo which is part of a “system” that includes conditioner and serum. And it’s yet another shampoo which has worked well on its own for many people. Researchers from the NIH and the University of Alabama, Birmingham have discovered a connection between the body’s innate immune regulation and hair graying. It was found that the transcritpion factor known as MITF, which plays an important role in melanocyte function, also plays a major role in hair graying. When the body’s immune system is dealing with a pathogenic infection such as bacteria or virus, molecules called interferons will send out signals to the body to take action against the pathogen. If MITF loses control of interferon response in melanocyte stem cells (due to an immune system response), hair turns gray. Essentially, immune system response may contribute to the process of hair graying according to the study.

Hair transplantation involves harvesting follicles from the back of the head that are DHT resistant and transplanting them to bald areas. A surgeon will remove minuscule plugs of skin that contain a few hairs and implant the plugs where the follicles are inactive. Around 15 percent of hairs emerge from the follicle as a single hair, and 15 percent grow in groups of four or five hairs.
NTU Working To Prevent Chemo-Induced Loss – Researchers from the National Taiwan University have developed a model for preventing chemotherapy induced hair loss, according to their publication in Cancer Research journal. The team, lead by professor Lin Sung-jan, identified a specific type of cell that hair follicles utilize to compensate for the toxicity which occurs during exposure to ionizing radiation (chemotherapy). These cells are called transit-amplifying cells (TAC). Preclinical animal testing with applied TAC-derived progenitor cells showed a 70-80% reduction in hair loss after chemotherapy and radiotherapy. Importantly, Sung-jan has recently stated he is in talks with companies about conducting trials on humans. It’s interesting to note that Lin Sung-jan has done an extensive amount of research on hair regeneration in the past. Hopefully this treatment could potentially be used for more common types of hair loss as well.
The most popular low-light laser therapy product is developed by Hairmax. They have a series of Hairmax Laser Combs. Treatment is applied weekly or more frequently. Notably, the Hairmax Laser Comb was the 3rd product to receive FDA approval for treatment of genetic balding. It followed Minoxidil and Finasteride. Though some scalps are very responsive to this laser treatment, others seem to only reap the benefit of slowing hair loss. Another downside is that each treatment session takes about 15 minutes, where the laser comb must be directed towards thinning and bald areas. Unless you have a handy device such as the one in the image, you stuck doing things manually.

There have also been studies on the effects of 1% pyrithione zinc shampoo and a 5% minoxidil solution. In one study, 200 men between the ages of 18 to 49 who experienced baldness between type III and type IV on the Norwood scale were given this treatment for a six-month period. They found that minoxidil, when used on its own, was approximately twice as powerful as pyrithione zinc at stimulating hair growth, but that both products were successful at increasing the amount of visible hair when used over a 26-week period.
The best fix by far for replacing lost hair is a transplant. Back in the day, docs used plugs that resembled cornrows (definitely not natural looking). Today, guys have more options. You can go for “the strip method” where a doctor surgically removes a strip of hair from the back of your head, dissects every hair graft under a microscope, and then plants the individual grafts onto hair-thin areas of your scalp with tiny incisions.

In most cases though, it's all a matter of attitude. Being bald should not be a cause of stigma. It does not make you less of a person or less masculine, less virile, and less attractive and appealing. In fact, a clean shaven head is becoming a popular trend among men these days, and there are certainly many bald men who have managed to make themselves look clean, elegant and suave despite their hair loss. Learn to come to terms with it. It's hair loss, yes, but not brain damage. It does not affect your core.
